Item 110 - Employee Last Name

This is the employee's legal surname and suffix, if any. The name on the Social Security card should agree with the name supplied on the PIMS transaction and EAR.

Instructions:

  1. If the employee has only one name, it must be entered in Item 110 - Employee Last Name. In Item 111 - Employee First Name and Middle Initial, press space bar once in the first position of the field.

  2. Do not enter spaces between punctuation or compound names, i.e. O'HARA, McCARTHY, BROWN-ORTEGA.

  3. Leave one blank space between multiple last names that are not hyphenated.

  4. Do not use any punctuation except an apostrophe (') or hyphen (-).

  5. Enter two spaces between surname and surname suffix or title. If using multiple suffixes or titles leave one space before the first suffix or title and two spaces before the second suffix or title.  When completing the PIMS transaction, add the symbol [] as a reminder to key the spaces (i.e. SMITH[][]SR).

More Info:

  1. Name changes are processed on the EAR document. The employee must provide a copy of form OAAN-7003 or other receipt issued by Social Security Administration. A copy of the form/receipt must be attached to the EAR document. Such employees should be encouraged to review the Designee for State Payroll Warrants (F243) and CalPERS beneficiaries.

  2. Corrections due to clerical error (misspellings, transpositions, etc.) are processed on the EAR; enter AGENCY ERROR in Item I. No OAAN-7003/receipt is required.
