New 2019-2020 CDIP Fellows need to complete the following steps to begin program participation.

What You Need To Do to Confirm Your Participation as a CDIP Fellow

The deadline for the following forms every year is August 18.

  1. Demographic Information Form
  2. Acknowledgements Regarding My Loan and What Happens If I Default
  3. Private Education Loan Applicant Self-Certification

    Forms 1-3 will be available through an Adobe Sign link we will email you.
  4. Submit a professional headshot (optional)
  5. Submit the Photography Release Form (optional)

What You Need To Do If Requesting Funds

  1. Submit your 2022-2023 Loan Request Form Online (Due August 18)
  2. Sign Loan Packet through Adobe Sign​ - October 27:
    1. Loan Disclosure Statement (emailed to you after submitting your loan request)
    2. Promissory Note
    3. Student Enrollment Verification Form(each term)
    4. Personal and Confidential Form

    3. Forms 1- 4 will be available through an Adobe Sign link we will email you. Full-time verification needs to be emailed separately to cdip@calstate.edu.
