4032.02  INCIDENT REPORTING

Presidents have been asked to report serious and fatal injuries or illnesses 
occurring on campus to the Office of the General Counsel, who informs the 
Chancellor and others as necessary.

In addition, a form has been issued for campus incident reporting, with 
instructions for the campus Public Safety department to notify the System 
Coordinator of Public Safety by telephone as soon as possible when a major 
incident has occurred, following up by completing the form and sending it to 
the System Coordinator.  See SUAM Appendix BA 4902-PS for a copy of the 
form.

When informed of an incident, the System Coordinator in turn notifies the 
Assistant Vice Chancellor, Business Affairs, who disseminates the 
information to concerned areas throughout the Chancellor's Office.

Note that a "major incident" is defined as any incident involving the 
campus, students, faculty, or staff which may be newsworthy or potentially 
sensitive:

1.	Crimes such as homicide, rape, robbery, assault, burglary (involving 
	large sums of money or valuables), arson, bombs, bomb threats where 
	buildings need to be closed, riots/disturbances, embezzlement of funds 
	or misuse of funds.

2.	Serious or fatal injuries or illnesses; suicides.

3.	Disasters or emergencies, such as fire, earthquake, power outage, 
	hazardous materials spill.
