2810.03  APPLICATION AND FISCAL OPERATIONS REPORT (ED 646-1)

The FISAP report is a combined fiscal operations report for the fiscal year just 
ended and an application for funds for the upcoming fiscal year.  This report 
covers the three campus-based Title IV programs---NDSL, SEOG, and CWS---
and is generally required to be submitted in late September or early October. 
Preparation of this report normally requires the joint effort of the Financial 
Aid Office and the Financial Aid Business Office.

The NDSL sections contain both annual and cumulative data, whereas the 
SEOG and CWS sections contain annual information.  The monetary data for 
the report are accumulated in the general ledger chart of accounts prescribed 
in SUAM Appendix BA 2992-ABS and the IBS Code Book.  Cumulative 
repayment information for the NDSL program reports the year-end (June 30) 
status of every NDSL loan made by the institution and is a summarization of 
defined segments of information in the subsidiary accounts.
