2156  INSURANCE CLAIMS

Reference:  ABS 86-21.

Property, liability, and loss of rental income insurance for all facilities owned 
by the housing system is purchased on behalf of The California State 
University by the Office of Insurance and Risk Management of the 
Department of General Services.  Premiums for these insurance policies are 
distributed to the campuses in accordance with procedures described in 
SUAM 2118.

Accidents causing serious bodily injury or major property damage should be 
immediately reported to Mr. Ralph Maurer, Chief, Office of Insurance and 
Risk Management at (916) 322-8971 and to Mr. Gene Forney, Business Services 
Specialist, Auxiliary Business Services at (310) 985-2790.  Accident Report 
Form 268 should be completed for all accidents causing injury or property 
damage and should be forwarded to the following addresses as soon as 
possible:
