The State Controller's Office has been requested to record the various systemwide charges and/or credits indicated for your respective campus on Plan of Financial Adjustment, Transaction Request TR039161 dated March 22, 2004. The attachment will give you the details of the original charges.

If you have questions regarding the charge or nature of the charge please contact the Chancellor Office contact. That person is listed on the Adnoat attachment and is listed directly below the charges.

Whenever possible we will also provide you with your campus contact who should be able to provide you with the correct account/accounts to be charged or credited. These will be either opposite the charge of your campus or on an additional attachment by campus.
