Subject: Form 204s for CSU Vendors
The CSU is obligated to have a Form 204 (Vendor Payment Record) on 
file for all active vendors.

APS program changes, planned for release in September 1993, will 
automatically make deductions from payments to vendors who have no 
Form 204 on file.  

Please take time to review the status of 204 forms with staff responsible for 
maintaining these files.  FRS Release 31 implements a modification to the 
APS program which will provide a Form 204 status report for all vendors.
