Subject: Update of Remittance Advices (CA 21)
The State Treasurer's Office has installed a new fax machine.  
The primary and alternate fax numbers have been changed on the 
instructions of the attached CA 21 Form (4/93).  The primary number 
is now (916) 654-5697.  Please make changes on your records.

The SCO has expressed concern about the readability of fax copies 
of remittance advices.  Please be sure your staff is using the best 
quality originals for faxing remittance advices.  The use of old 
photocopied forms may not produce faxes that are readable by the 
SCO staff and could delay the remittance process. 

A camera-ready copy of the form (front and back) is attached, together 
with several back-to-back copies, for your convenience.
