The CSU QI Program creates, administers and performs analysis for various departments both at the Chancellor's Office and at the campuses. Once a request is received using the survey request form, the requestor will be contacted to confirm the requirements.

All surveys are compliant with ADA Section 508 accesibility standards.

Please note that a survey request form MUST be filled out and submitted for all requested surveys. Also, a two-week lead time is required from the initial request date to the target start date.
