Health Care Flexible Benefits Programs Forms Open Enrollment
An annual open enrollment period each Fall allows employees to enroll in or make changes to health plans. For 2009, Open Enrollment begins September 14 and ends October 9, 2009. For more information about Open Enrollment, read the 2009 Open Enrollment Newsletter.
  • Covers medical, dental, Dependent Care Reimbursement Account (DCRA), Health Care Reimbursement Account (HCRA) and FlexCash. To make changes complete the Open Enrollment Benefits Worksheet (.xls | .pdf) and Declaration of Health Coverage and and return them to HR Services by October 9, 2009.

  • HCRA and DCRA require annual re-enrollment (even if there are no changes) to maintain coverage. To enroll or re-enroll, complete the HCRA/DCRA Enrollment Form.

  • Changes made during open enrollment take effect the following January 1, 2010.

COBRA

Retiree Health (Medical and Dental)
