3. If your answer to question 2 is "NO", in what MARC field is the OCLC number stored in your local system

4. Indicate what data, if anything, is stored in MARC field 001 in your local system

5. In your local system does the OCLC number have a prefix? YES NO

If "YES", please attach a print-out of an example of an OCLC number with prefix as stored in your local system.

6. In your local system, do you store anything in MARC field 035? YES NO

If "YES," describe what is stored in that field and attach an example

7. What status messages are being used in your local catalog, e.g. "Not checked out," "missing," etc. Please list or attach all status messages you are currently using:

8. Are there any bibliographic records from your local system that should not be loaded into the UIAS? YES NO

If "YES," describe what records and how these records can be identified, e.g. personal copies on reserve

9. Where is your library storing serial summary holdings? Please describe in which type of record and in which field tag, e.g. in bibliographic record, field 850. Please print and attach example.
