Subject:General Fund-Support -- 1994/95 Mid-year Financial Status Report
The purpose of this letter is to request the completion of the 1994/95 Mid-year
Financial Status Report. The report format utilizes financial information
from the Financial Accounting Systems' Consolidated Data Base (CDB).
Attachment A is the only form requiring completion. The other attachments
provide documentation and an audit trail for each dollar amount shown.
These amounts can be reconciled to the SAM 6B report that the campuses run
Attachments B and C contain detailed instructions regarding completion of
the report and documentation for the information from the CDB. Please
distribute the attachments to the appropriate staff on your campus.
Attachment D is a listing of the Allocation Orders that have been processed as
of 12/31/94 for your campus. Attachment D is included as an information
item to allow the campus to reconcile the budget amounts shown on the CDB
report with the amounts maintained by our office.
Please transmit Attachment A with a cover memo explaining any deficits,
outstanding items, missing transactions, suspense amounts, etc., to this office
by Friday, March 3, 1995. The report should be approved and signed by the
campus Vice President for Administration in the space provided.
