Current Fellow Forms

Every GTF Fellow MUST fill out one of the following forms for the Fall Status Update. Click on the name of a form to bring up the form and its coversheet.

PLEASE NOTE THAT WE ARE NOT OFFERING SPECIAL DEFERMENT!

  • If you are CURRENTLY ENROLLED FULL TIME for the Fall term in a teacher preparation program (NOT EMPLOYED), complete Form G (pdf, 61k).

  • If you have COMPLETED your credential program and have SECURED EMPLOYMENT, complete Form H (pdf, 44k).

  • If you are presently IN DEFAULT or were granted or wish to request MEDICAL or MILITARY DEFERMENT, complete the Fall Status Update Form (pdf, 23k).

  • If NONE OF THE ABOVE describes your current status, complete the Fall Status Update Form (pdf, 23k).
